The Parliamentary and Health Service Ombudsman, Julie Mellor, has raised her concerns about poor co-ordination between health and social care, which, she feels has led to vulnerable people falling through the gaps.

The case that she has cited involved a vulnerable young man who suffered from psychiatric problems, who was wrongly charged more than £8000 for accommodation, put at risk of financial exploitation and was left without bathing facilities for six months. The patient was released from a psychiatric unit, but the monitoring of his care following his discharge was so poor that it amounted to service failure. Her concern is that no one took responsibility for co-ordinating his care properly and that opportunities put put things right were denied because he had “woefully insufficient aftercare plan reviews… he only had three in five years, when the Mental Health Act Code of Practice states that he should have had ten”.
